PERF: Async notify users after inviting group (#12697)
Inviting a group generates a notification for each member. If this
happens synchronously it may take a while, leading to a poor user
experience.

FEATURE: Mailing list mode default disabled (#11091)
Mailing list mode can generate significant email volume, especially on sites with a large user base. Disable mailing list mode via site settings by default so sites don't experience an unexpectedly large cost from outgoing email.

9a9e31f927 FIX: properly set notification levels on group invite
Previously we relied on side effects to set tracking state correctly
when inviting groups to messages

Also has a minor optimisation in that we use pluck instead of pulling in
full record

* Introduced fab!, a helper that creates database state for a group

It's almost identical to let_it_be, except:

 1. It creates a new object for each test by default,
 2. You can disable it using PREFABRICATION=0

4ea21fa2d0 DEV: use #frozen_string_literal: true on all spec
This change both speeds up specs (less strings to allocate) and helps catch
cases where methods in Discourse are mutating inputs.

Overall we will be migrating everything to use #frozen_string_literal: true
it will take a while, but this is the first and safest move in this direction

fa5a158683 REFACTOR: Move queue_jobs out of SiteSetting
It is not a setting, and only relevant in specs. The new API is:

```
Jobs.run_later!        # jobs will be thrown on the queue
Jobs.run_immediately!  # jobs will run right away, avoid the queue
```

d1d9a4f128 Add new run_jobs_synchronously! helper for tests
Previously if you wanted to have jobs execute in test mode, you'd have
to do `SiteSetting.queue_jobs = false`, because the opposite of queue
is to execute.

I found this very confusing, so I created a test helper called
`run_jobs_synchronously!` which is much more clear about what it does.

* FIX: don't allow inviting more than `max_allowed_message_recipients` setting allows

* add specs for guardian

* user preferences for auto track shouldn't be applicable to PMs (it auto watches on visit)

Execlude PMs from "Automatically track topics I enter..." and "When I post in a topic, set that topic to..." user preferences

* groups take only 1 slot in PM

* just return if topic is a PM

5f64fd0a21 DEV: remove exec_sql and replace with mini_sql
Introduce new patterns for direct sql that are safe and fast.

MiniSql is not prone to memory bloat that can happen with direct PG usage.
It also has an extremely fast materializer and very a convenient API

- DB.exec(sql, *params) => runs sql returns row count
- DB.query(sql, *params) => runs sql returns usable objects (not a hash)
- DB.query_hash(sql, *params) => runs sql returns an array of hashes
- DB.query_single(sql, *params) => runs sql and returns a flat one dimensional array
- DB.build(sql) => returns a sql builder

7fc8a36529 DEV: Take 2 Queue jobs in tests by default.
On my machine this cuts the time taken to run our test suite
from ~11mins to ~9mins.

29fac1ac18 PERF: improve performance of unread queries
Figuring out what unread topics a user has is a very expensive
operation over time.

Users can easily accumulate 10s of thousands of tracking state rows
(1 for every topic they ever visit)

When figuring out what a user has that is unread we need to join
the tracking state records to the topic table. This can very quickly
lead to cases where you need to scan through the entire topic table.

This commit optimises it so we always keep track of the "first" date
a user has unread topics. Then we can easily filter out all earlier
topics from the join.

We use pg functions, instead of nested queries here to assist the
planner.

c531f4ded5 remove rails-observers
Rails yanked out observers many many years ago, instead the functionality
was yanked out to a gem that is very lightly maintained.

For example: if we want to upgrade to rails 5 there is no published gem

Internally the usage of observers had quite a few problem.

The series of refactors renamed a bunch of classes to give us more clarity
and removed some magic.

3e50313fdc Prepare for separation of RSpec helper files
Since rspec-rails 3, the default installation creates two helper files:
* `spec_helper.rb`
* `rails_helper.rb`

`spec_helper.rb` is intended as a way of running specs that do not
require Rails, whereas `rails_helper.rb` loads Rails (as Discourse's
current `spec_helper.rb` does).

In this commit, I've simply replaced all instances of `spec_helper` with
`rails_helper`, and renamed the original `spec_helper.rb`.

This brings the Discourse project closer to the standard usage of RSpec
in a Rails app.

At present, every spec relies on loading Rails, but there are likely
many that don't need to. In a future pull request, I hope to introduce a
separate, minimal `spec_helper.rb` which can be used in tests which
don't rely on Rails.

  When enables users can elect to watch everything by default
FIX: Custom email subjects (x quoted you in [title], x replied to [title])
  was removed, this broke email grouping. TBD, include info in footer somehow
FIX: topic user specs were messy, reduce side effects
