completions/service: Fix output on OpenRC systems
...
This used `type -f`, which prints, and only silenced stderr.
Detected by running the check-completions test on Alpine.
It appears nobody does that.

Make __fish_is_nth_token and __fish_nth_token private again

Their names are not perfect, so let's keep them as internal functions,
until we figure out how/if we want to expose this.

We keep __fish_is_nth_token for compatibility and edit the
implementations of __fish_is_nth_token, __fish_is_first_token and
__fish_is_token_n to use fish_is_nth_token

Fix service completions on non-FreeBSD platforms

I never realized script-local variables set after the function
definition do not keep their value when the function is executed later
in the file.

Fix completions for service
...
I'm not sure what was up with the old completions,
`$__fish_service_commands` is not set anywhere and completions for the
command (not the service) were not being generated on my machine.

Disable php completion for the 4th part of the command, since php-eselect is differently designed and causes errors

Drop use of the N_ no-op for translation descriptions. Instead, tell xgettext that any token following '--description' should be translated. This should greatly speed up completion loading on platforms where fork() is slow. (Hi, OS X)
